An Institute of Medical Coding guide to understanding how accurate documentation supports sound coding decisions.
Why clarification is needed
A record may contain conflicting diagnoses, unclear relationships, missing specificity or a term that cannot be coded as written. A provider query helps establish what the clinician intended to document when the answer cannot be determined from the record.
Recognize a genuine gap
First review the full encounter and relevant coding instructions. A query should address a specific ambiguity that affects accurate reporting. Do not query simply because a more highly paid diagnosis might be possible.
Keep the question neutral
Present the relevant clinical facts and ask the provider to clarify in their own clinical judgment. Follow the organization’s approved query process and retain the response in the appropriate record. A coder should not invent a diagnosis or steer the provider to an unsupported answer.
What students should remember
Document the uncertainty, the source of conflicting information and the reason clarification matters. Practice distinguishing a missing code lookup from missing clinical documentation: they are different problems.
